Only one name per form.

Make check payable to PSHOG $24 (individual) or $40 (couple).

Bring your completed application and fee to our next Chapter Meeting or other PSHOG Event.
PSHOG Chapter Meetings are held every third Wednesday of the month, at 7:00PM at the Palm Springs H-D dealership.
